Preparing For Arrangements

We’ve put together this list of considerations to hopefully alleviate some of the stress involved in making arrangements after you’ve lost a loved one.  Here are some things you can gather in preparation for the funeral or cremation.  

The Pennsylvania Department of Health Requires The Following Information for The Death Certificate:

Full Name

Date of Birth

Place of Birth
(City, State, County)

Education Level

Usual Occupation

Social Security Number

Mothers First and Maiden Name

Fathers Full Name

Marital Status

Veteran Status (Yes/No)

The Following Information/Items Will Allow Us To File Claims for Death Benefits and Prepare for the Funeral/Cremation:

Descedents Insurance Policy (If Applicable)

Cemetery Information

DD-214 (For Veterans)

A Favorite Photo for Obituary

Clothing for Burial or Cremation

Dentures (If Applicable)

Jewelry (Optional)
